Wisconsin Partnership Program accepting applicants for Community Impact Grant Program

The Wisconsin Partnership Program at the UW School of Medicine and Public Health has released its 2023 Community Impact Grant Program Request for Partnerships.

The Community Impact Grant Program supports community partnership initiatives that address the social determinants of health to improve health and advance health equity in Wisconsin’s urban and rural communities.

This year’s Community Impact Grant Program request for partnerships offers two grant award levels:

  • Level One is designed for small to medium sized organizations with annual expenses less than $500,000.
  • Level Two is designed for larger organizations with annual expenses of $500,000 or more.

Each grant level has a maximum award amount for up to three years; up to $250,000 for Level One and up to $500,000 for Level Two. The Wisconsin Partnership Program’s Oversight and Advisory Committee anticipates allocating up to five awards within each grant level.

Since 2015, the Wisconsin Partnership Program has allocated $32 million in Community Impact Grants to support community partnerships across a wide range of health initiatives, communities and geographic areas.
