This project, Building a Caregiver Wellness and Resource Hub for Dementia Families in Milwaukee, led by Legacy Home and Respite Care Foundation seeks to strengthen its organizational capacity by transforming its website into a centralized Caregiver Wellness and Resource Hub for family caregivers of people living with dementia. Building on programs that have directly served the community, the project aims to create a more accessible, caregiver-centered digital experience. The enhanced hub is designed to streamline access to respite services, Caregiver Sanctuary programming, support groups, wellness resources and event registration while improving backend operations by consolidating forms, reducing manual workflows and strengthening staff capacity to manage the site for long-term sustainability.

The Challenge

Legacy Home and Respite Care Foundation has built trusted, community-rooted programs that support family caregivers of people living with dementia, directly serving caregivers and reaching more than 75,000 individuals through community outreach and education. As demand for these services has grown, the organization’s digital infrastructure has not kept pace with that growth. Caregivers, who often balance employment, parenting and financial responsibilities while experiencing high levels of stress, social isolation and burnout, must navigate multiple disconnected systems, emails, links, forms and manual processes to access support. This fragmentation creates barriers for caregivers and increases administrative burden for staff, whose time is increasingly spent on manual coordination, follow-up and data tracking rather than direct caregiver support.

Project Goals

To better meet the needs of underserved caregivers across Milwaukee County, this project identified the need for a centralized, user-friendly digital hub that would improve service navigation, operational efficiency and continuity of care. To accomplish this, the organization will:

  1. Strengthen their organizational capacity by developing a centralized, interactive Caregiver Wellness and Resource Hub that improves caregiver access to services and reduces administrative burden.
  2. Improve caregiver experience by creating an intuitive, mobile-friendly digital system that supports engagement, navigation, and continuity of care.
  3. Build internal staff capacity to independently manage and sustain the digital hub over time.
